Be Ready for Your Annual Review! Under the Illinois Administrative Code, Illinois Supportive Living Providers are subject (at least annually) to an on-site review to ensure that the community is compliant with the Supportive Living Program requirements. Recently, the Illinois Department of Healthcare and Family Services issued a revised Annual Survey Tool to use during the onsite review process.
