Be Ready for Your Annual Review!
Under the Illinois Administrative Code, Illinois Supportive Living Providers are subject (at least annually) to an on-site review to ensure that the community is compliant with the Supportive Living Program requirements. Recently, the Illinois Department of Healthcare and Family Services issued a revised Annual Survey Tool to use during the onsite review process. 

Join us for a special opportunity to learn more about the changes to the Survey Tool and Process as well as earn 2 CEU credits for nursing, social work, and nursing home administrators.

    Each workshop gives attendees the opportunity to earn a maximum of 2.0 CE hours per workshop. All other attendees will receive a Certificate of Participation.

Credit

SIU School of Medicine is pre-approved as a continuing nursing education provider pursuant to Section 1300.130, subsection c), 1), B) and P) of the Illinois Department of Financial and Professional Regulation Nurse Practice Act. Nurses may receive a maximum of 2.0 contact hours for completing this activity.

 SIU School of Medicine is a licensed provider for continuing education for nursing home administrators (license #139.000027), social workers (license #159.000106), professional/clinical counselors (license #197.000073), dietitians/nutrition counselors (license #199.000121), and clinical psychologist (license #268.000008). This program offers a maximum of 2.0 CE hours. All other attendees will receive a Certificate of Attendance.
